Job summary

Guardian Angel Home Care is seeking an experienced Medical Social Worker (MSW) to join our home health team on a PRN / Per Diem basis. This role offers flexibility, supplemental income, and strong compensation without a full-time commitment.

Expect high per-visit compensation and possible hourly options, with assignments based on patient census and service area needs. Travel to patients’ homes is required, and candidates must hold an MSW with a full, active, unrestricted license.

Qualifications

• Master's Degree in Social Work (MSW) is REQUIRED.
• Full, active, unrestricted social work license is REQUIRED.
• Limited, temporary, associate, provisional, or restricted licenses will not be accepted.
• Experience in home health, hospice, hospital, rehabilitation, or community-based settings preferred.
• Strong knowledge of community resources.
• Excellent communication and documentation skills.
• Ability to work independently in patients’ homes.
• Reliable, compassionate, and professional.

Responsibilities

• Complete psychosocial assessments.
• Evaluate patient and family needs.
• Provide counseling and emotional support.
• Connect patients with community resources and support programs.
• Assist with financial, transportation, housing, and long-term care resources.
• Support patients and families with adjustment to illness or disability.
• Participate in interdisciplinary care planning.
• Coordinate with nurses, therapists, physicians, and other clinical staff.
• Document visits and interventions accurately and timely.
• Travel to patients’ homes throughout the assigned service area.
