Job summary

Prime Home Health is seeking a compassionate LVN for in-home patient visits on a PRN basis in Jourdanton and surrounding areas. This role requires living in or near Jourdanton/Pleasanton to visit patients in person and collaborate with an interdisciplinary team.

Duties include direct patient care, assessments, documentation, and patient education, with a focus on comfort and safety. Applicants should have Texas LVN license, CPR/BLS, and at least 1 year of in-home LVN experience, plus proficiency

Qualifications

• Valid LVN license issued by the State of Texas.
• Must live in Jourdanton, Pleasanton, Poteet or surrounding areas.
• Minimum of one year of LVN in-home patient visits.
• Experience using Kinnser/WellSky EMR; comfortable with apps like Slack.

Responsibilities

• Provide direct patient care per plan of care, including meds, treatments, vital signs and symptom management.
• Conduct assessments, document observations, and maintain medical records.
• Educate patients and families about conditions, treatment options, and end-of-life care.
