Job summary

Momentous Healthcare is seeking an LPN for a night shift (7P-7A) at our Sidney, OH location. The position focuses on compassionate care for residents, administering medications, and supporting a welcoming, team-driven environment.

The ideal candidate holds an LPN license and has prior experience with the elderly population, preferably in long-term care. Strong empathy, effective communication, and a patient-first mindset are essential.

Qualifications

• Current State License as an LPN.
• Prior experience working with the elderly population.
• Prior experience in Long-Term Care/Healthcare is preferred.
• Empathy and care for those that need help.
• Effective communication skills.

Responsibilities

• Administer medications and treatments to residents as per physician orders.
• Maintain accuracy of patient records including charting as needed.
• Report and investigate incidents thoroughly and timely.
• Assist physicians during rounds.
• Provide guidance and training for aides.
• Demonstrates proficient nursing skills.
