Job summary

Laurelwood Health Care Center in Jackson, Tennessee, is seeking compassionate, reliable LPNs to join our care team on a PRN basis. You will direct the daily activities of CNAs, participate in admissions, discharges and transfers, and help assess residents and administer medications per physician orders.

Current, unrestricted LPN license for Tennessee and CPR/BLS certification are required. We value teamwork, confidentiality, and a flexible, patient‑centered approach to care.

Qualifications

• Current unrestricted LPN License for the state of Tennessee.
• Current CPR/BLS Certification.

Responsibilities

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident's conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report any changes in a resident's condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
• Perform other assigned duties given by clinical or administrative leadership.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.
