Job summary

Grafton Integrated Health Network is seeking a Licensed Practical Nurse (PRN) to join a compassionate team delivering evidence-based care. The LPN will provide nursing services to clients, monitor health, document care, and report to the nurse manager.

You will administer medications, assist RNs or physicians, participate in MDT meetings, and support crisis intervention. The role offers day and overnight hours and opportunities for growth within a supportive, tobacco-free workplace.

Qualifications

• Requires a high school diploma or equivalent; Associates degree preferred.
• Valid LPN license (State Board of Nursing).
• 1 year or more of practical nursing in a behavioral health setting or related field.
• Strong communication, computer proficiency, and ability to collaborate with staff.

Responsibilities

• Assess and monitor client health, collect vitals and data.
• Assist RNs/physicians in delivering direct patient care.
• Administer medications as directed.
• Document care in patient records promptly and accurately.
• Participate in MDT meetings and contribute to care plans.
• Provide crisis intervention as needed.
• Attend meetings, trainings, and on-call duties.
