Laurelwood Health Care Center is seeking a compassionate LPN to join our care team in Jackson, Tennessee. This PRN role supports daily resident care, medication administration, and care plan updates within a team-driven environment.

You will supervise CNAs, assist with admissions/discharges, monitor residents’ conditions, and communicate changes to the nurse supervisor while upholding HIPAA and care standards.

Formación

• Current unrestricted LPN License for the state of Tennessee.
• Current CPR/BLS Certification.

Responsabilidades

• Direct the daily activities of CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess residents' conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
• Perform other assigned duties given by clinical or administrative leadership.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.
