Job summary

Del Corazon Hospice seeking a compassionate PRN Hospice Social Worker to serve Santa Fe and surrounding Northern New Mexico communities. The role focuses on psychosocial assessments, counseling, and care planning as part of the hospice team.

Requirements include an MSW and current LMSW license in New Mexico, with flexible PRN scheduling and mileage reimbursement offered. Prior hospice or healthcare experience is preferred.

• Provide initial social services assessments to hospice patients.
• Provide emotional support and counseling to patients and families.

We are seeking a compassionate and dependable PRN Hospice Social Worker to join our growing team serving Santa Fe and surrounding Northern New Mexico communities.

Responsibilities:

• Provide initial Social Services assessments to hospice patients

• Provide emotional support and counseling to patients and families

• Assist with psychosocial assessments and care planning

• Support patients and caregivers with community resources and end-of-life planning

Qualifications:

• MSW required

• Current New Mexico social work license required (LMSW)

• Hospice, home health, palliative care, or healthcare experience preferred

• Strong communication and interpersonal skills

• Valid driver’s license and reliable transportation
