Job summary

Cantex is seeking a Licensed Master Social Worker (PRN) to identify the social and emotional needs of patients in hospice care. You will perform assessments, develop care plans, and coordinate with the care team to support discharge planning.

The role requires experience in hospice or long-term care, strong communication, documentation, and analytical skills, and the ability to educate patients and families while ensuring quality care across settings.

Qualifications

• Licensed Master Social Worker per state requirements or eligible for licensure.
• Masters degree in Social Work or in a human services field.
• Has at least one year of experience in hospice, long-term care setting, and/or related healthcare setting.
• Skills in communication, documentation, assessment, and social work methods and techniques.
• Ability to read, write, and speak the English language.
• Reliable transportation. Valid and current auto liability insurance.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients’ psychosocial needs.
• Apply admission and re-certification criteria for hospice care.
• Develop and document plans of care; revise as needed for quality and continuity.
• Educate patients and families on disease process, self-care, end-of-life care, nutrition and dietary needs; train staff as needed.
• Determine scope and frequency of services based on acuity and family needs.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Coordinate infection control and standard precautions.
• Direct Hospice Aide and LPN/LVN to ensure quality care.
• Prepare progress notes showing progress toward goals and ensure documentation.
• Notify physicians of changes in patient needs and outcomes; monitor response to care.
• Monitor cases for third party payor compliance; uphold professional standards.

Job Summary:

The overall purpose of the Licensed Master Social Worker position is to enable the facility to identify the medically related social and emotional needs of the patients/residents. This position acts to meet those needs through individual and group social services programming. This position also promotes patients'/residents potential during their stay and collaborates with physician and patient/resident in preparation for discharge.

Essential Functions:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs, (i.e., risk for grief, cultural and spiritual, verbal and non‑verbal).
• Apply specific criteria for admission and re‑certification to hospice care to establish appropriate levels of care and the patient's eligibility.
• Implement/develop/document the plan of care to ensure quality and continuity of care and recommend revisions to the plan as necessary.
• Consult with and educate the patient and family regarding disease process, self‑care techniques, end‑of‑life care, nutrition and dietary needs. Prioritize any needs with the members of the IDG. Provide training to other staff as needed.
• Determine scope and frequency of services needed based on acuity and patient/family needs. Assess the ability of the care giver to meet the patient's immediate needs upon admission and throughout care.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Applies concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
• Provide clinical directions to the Hospice Aide and LPN/LVN to ensure quality and continuity of service provided.
• Prepare clinical and progress notes that demonstrate progress toward established goals. Ensure continuity of quality patient care delivered with appropriate documentation.
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient and family response to care.
• Monitor assigned cases to ensure compliance with requirements of third party payor.
• Demonstrate commitment, professional growth and competency.
• Promote Agency philosophy and administrative policies.
• Provide effective communication to patient, family, team members, and other health care professionals.
