Responsibilities

The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs skilled technical care in the patient's home, within the scope of practice defined by state laws. Responsibilities include:

• Providing services according to the plan of care and in coordination with the healthcare team, without altering the plan of care.
• Instructing family members on patient care and maintaining a healthy environment.
• Observing, recording, and reporting patient responses and condition changes to the supervising nurse or physician daily.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice.
• Valid Driver's License, vehicle insurance, and access to reliable transportation.
• CPR Certification required.

Additional State-Specific Requirements:

• LA: Current LPN licensure in LA without restrictions and at least one year of LPN experience.
