The role requires an active LPN license and BLS, with inpatient hospital experience preferred. You will work in a dynamic, patient-centered environment focused on safety, learning, and professional growth.

Qualifications

• Current state LPN license and BLS certification are required.
• Two years of inpatient hospital experience; rehab experience preferred.
• Strong knowledge of infection prevention and patient care standards.

Responsibilities

• Provide skilled nursing care to patients as ordered by physician under RN supervision.
• Implement medication and treatment orders according to care plan.
• Contribute to individualized patient care plans with RN input.
• Evaluate intervention effectiveness and report patient care issues.
• Document patient care and report problems to supervisor.
• Identify patient learning needs and reinforce self-care techniques.
• Maintain safety and infection prevention standards.
• Participate in scheduling and adapt to staffing emergencies.
