Job summary

Collage Home and Community Rehabilitation Partners in El Cajon, CA is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in patients' homes and surrounding communities. Qualifications

• Master's degree in Social Work is required.
• State licensure as an LCSW is required.
• Excellent planning, organization, writing, and time management skills are required.

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
