Collage Home and Community Rehabilitation Partners in El Cajon, CA is seeking experienced Licensed Clinical Social Workers to provide truly patient-centered care in the home and surrounding communities. PRN positions available to support patients during their stay.

Our interdisciplinary teams develop individualized treatment plans for neuro and ortho conditions, with an emphasis on resource linkage, adjustment to disability, and family communication.

Qualifications

• Completion of a Master's degree in Social Work and state licensure as an LCSW are required.
• Excellent planning, organization, writing, and time management skills are required.
• Ability to communicate with families, professionals, and co-workers is a must

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
