Job summary

ADDAX HOME HEALTH LLC in Florida is seeking a compassionate Occupational Therapist to join our PRN team. You will meet with multiple patients daily, assess their needs, and craft personalized therapy plans while coordinating with their health care team.

You’ll educate patients on lifestyle changes, maintain detailed records, and support follow-up care across Lee, Collier, Charlotte and Glades Counties. Flexible work-from-home options are available, with pay per visit discussed.

Qualifications

• Must hold the required state license for occupational therapy.
• Strong communication and interpersonal skills.
• Excellent organizational skills.
• Good attention to detail.
• The ability to stay positive and compassionate over a long period of time with patients.

Responsibilities

• Meet with patients from many different backgrounds and discuss their health concerns and therapy needs.
• Conduct evaluations to determine a starting point for each patient.
• Create a therapy plan specific to each patient that will help them meet their mobility goals.
• Work closely with other members of each patient’s care team to provide adequate follow-up care.
• Maintain excellent records of patient therapy and progress (we use WellSky Kinnser).

Job Summary

We are seeking an Occupational Therapist to join our team as a PRN Occupational Therapist. You will be meeting with several patients in a day, discussing their treatment needs and ultimate goals, and crafting customized, comprehensive therapy plans for them. You will also work closely with other members of their health team to arrange follow-up care, help educate patients on lifestyle changes they may need, and provide a positive, affirming presence in their lives. The ideal candidate has a compassionate style of care, has strong communication and interpersonal skills, and is comfortable handling multiple different cases at a time. We service Lee, Collier, Charlotte and Glades Counties. Pay per visit to be discussed; we have very competitive rates.
