Job description

Job Summary

An Infusion Nurse I is a licensed registered nurse who provides basic direct patient care to patients in the home or an alternate infusion site under close supervision, ensuring patient safety and continuity of care.

Responsibilities
  • Initiate, develop, and implement basic nursing plans of care, evaluating patient progress toward goals.
  • Provide direct patient care by administering treatments, medications, and educating patients and caregivers.
  • Modify treatment plans in response to changing patient status based on physician orders.
  • Assess patient needs and physical status at each visit, and re‑evaluate through physical reassessment and response to therapy.
  • Obtain and clarify physician orders, inform physicians promptly of significant changes, and provide written summary within supplemental order.
  • Teach patients and families using patient‑centered approaches to achieve independence with prescribed therapy.
  • Communicate effectively with the clinical team, medical providers, patients, and families to facilitate continuity of care.
  • Prepare real‑time clinical documentation during visits and submit to the nursing department in compliance with billing timelines.
  • Co‑ordinate discharge planning, prepare discharge summaries, and report patient care needs upon transfer.
  • Ensure compliance with agency policies, professional standards, regulations, and accreditation standards.
  • Utilize computer and automation technologies to validate orders, record care, and follow assigned visit schedules.
  • Respond promptly and appropriately to patient requests and emergencies.
  • Maintain ethical and professional conduct, safeguard patient confidentiality, and adhere to HIPAA.
  • Provide oversight of paraprofessionals and act as supervisor when needed.
  • Participate in multidisciplinary team conferences, precepting, training, and mentoring of other nurses.
  • Support nursing department on‑call responsibilities, and perform other duties as directed.
Qualifications
  • Active, unrestricted RN license in the state of practice.
  • 0–2 years of healthcare related experience.
  • Current CPR certification.
  • Demonstrated competency in patient‑care standards and infusion skill sets.
  • Basic knowledge of computer operating systems and software.
  • Valid driver’s license and access to a vehicle with proof of liability insurance.
  • Physical ability to lift up to 75 pounds, walk, stand, crouch, kneel, and perform routine tasks.
Work Requirements
  • Willingness to travel up to 100% of the time for business purposes.
  • Ability to work in various home environments, including exposure to bodily fluids and hazardous substances.
