Venture I Inc. in Austin, TX is seeking an experienced, licensed Speech Therapist to join our home health team on a PRN, per-visit basis.

You will travel to patients’ homes, assess speech needs, and create personalized treatment plans to help patients reach their goals. The ideal candidate holds a CCC-SLP, demonstrates strong communication and organizational skills, and is comfortable working independently in a home setting.

Qualifikationen

• Active state license for practicing speech therapy.
• CCC-SLP from ASHA or eligible.
• Strong communication and interpersonal skills.
• Excellent organizational skills.
• Strong charting and documentation skills.
• Familiarity with Microsoft Office programs.
• Home health experience preferred.
• Reliable transportation, valid driver's license, and current auto insurance.
• Current CPR certification.
• Ability to pass background check and drug screening.

Aufgaben

• Meet with patients for the first time in their home, answering introductory questions.
• Make evaluations and create treatment plans tailored to the patient, setting short and long-term goals.
• Assist in implementing the treatment plan, adjusting as needed for outcomes.
• Maintain records of patient needs, care, and outcomes, including timely EMR documentation for each visit.
• Work closely with the family and patient to ensure knowledge and tools for success.
• Communicate patient progress and changes to the interdisciplinary team and referring physician.
• Complete recertification and discharge documentation as needed.
• Maintain compliance with Medicare/Medicaid home health regulations and agency policies.
