Job summary

Halcyon Home in Austin, TX, is hiring a PRN Home Health Speech-Language Pathologist to support our geriatric population. You can pick your own hours and join a supportive team that values compassionate patient care.

The role involves ongoing clinical assessments, planning and implementing care, documenting thoroughly, and providing speech-language pathology services in patients' homes. A Master’s degree and state licensure are required, with home health experience preferred.

Qualifications

• Master's degree in Speech-Language Pathology required.
• State licensure as a Speech-Language Pathologist.
• Experience in home health care preferred.
• Proficiency with Homecare Homebase software is a plus.
• Strong communication and interpersonal skills.

Responsibilities

• Performs ongoing clinical assessment, including OASIS assessments, to identify patient needs and determine level of functioning.
• Develops and evaluates the plan of care with patient, representative, and caregiver(s).
• Prepares and completes clinical notes, phone orders, and documentation timely.
• Analyzes patient needs and provides care ordered by physician as indicated in the plan of care.
• Revises the POC based on evaluation data.
• Provides therapeutic treatments for speech, language, cognitive, and swallowing disorders with appropriate tests and measurements.
• Provides services ordered by physician as indicated in the plan of care.
• Applies infection control and universal precautions in patient care activities.
• Accepts assignments consistent with education and competence to care for patients.
• Communicates with physicians and other health care practitioners involved in the plan of care.
• Provides patient and caregiver education.
• Uses effective interpersonal relations to achieve desirable outcomes.
• Meets mandatory continuing education requirements of the Agency and licensing board.
