Job summary

PruittHealth Corporation is seeking a PRN Speech Therapist/SLP for Home Health Services to evaluate, treat, and consult with patients in their homes. The role emphasizes collaborative care and adherence to care plans crafted with the interdisciplinary team.

Responsibilities include completing evaluations, developing plans of care with timely documentation, coordinating with physicians and families, and ensuring compliance with regulatory standards while delivering compassionate patient care.

Qualifications

• Must hold a current ASHA-certified Speech-Language Pathologist credential.
• Graduate from ASHA-accredited program.
• If foreign-trained, the program must be deemed substantially equivalent to a US SLP program by a credentials evaluation organization approved by ASHA.

Responsibilities

• Complete evaluations, assessments, and treatments per policy and care plan timelines.
• Collaborate with interdisciplinary team and physician to develop and manage the plan of care.
• Demonstrate home-care competency for patient-focused skills and care delivery.
• Prioritize patient needs, exercise initiative, and meet outcomes within deadlines.
• Assist with training and orienting other staff or trainees as needed.
• Utilize approved patient/family education materials in care delivery.
• Report suspected compliance issues to supervisor until resolved.
• Adhere to corporate compliance program and regulatory guidelines.
• Meet agency productivity standards and manage time/resources efficiently.
• Participate in surveys, in-services, case conferences, staff meetings, and quality programs.
