Job summary

Interim Healthcare is seeking a PRN Home Health RN Case Manager to serve the Greater Houston area. You will supervise patient care from admission to discharge, coordinate with the care team, and perform visits including admissions, recertifications, and discharges.

Requirements include Texas RN license, OASIS training, a valid driver’s license, and willingness to travel. Travel throughout the assigned territory is expected.

Qualifications

• Graduation from an accredited nursing school with RN license in Texas.
• Experience in home health nursing preferred (1 year).
• Willingness to travel throughout assigned territory.

Responsibilities

• Supervise and manage patient care from admission to discharge.
• Coordinate patient care with clinical staff and providers.
• Perform home health visits, re-certifications, and discharges.
• Record, assess and monitor patient progress and symptoms.
• Develop care plans based on assessment and needs.
• Check vital signs, administer medications, and dress wounds.
• Educate patients per physician recommendations.
• Document care according to guidelines.
