Job summary

You'll perform comprehensive OASIS assessments, develop and adjust care plans, and educate patients and families. Weekend/holiday on-call rotations apply; potential for full-time growth as demand rises.

Qualifications

• Graduate of an approved nursing program with an active Florida RN license.
• Prefers BSN or MSN degree.
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills.
• Will­ingness to participate in weekend and holiday on-call rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
• Flexible scheduling and manageable caseloads.
• Growth potential: several roles could expand to full-time as the market grows.
