Job summary

Interim HealthCare in Palm Beach, FL is seeking a PRN Home Health Registered Nurse (RN) to provide one-on-one care in patients' homes. This PRN role offers flexible scheduling and the opportunity to make meaningful, patient-centered differences with autonomy in care planning.

You will work with a hospital- or facility-based team, perform assessments, document progress, educate patients and families, and coordinate discharge.

Qualifications

• 1 year of home healthcare experience required.
• Oasis experience required.
• Graduate of an accredited nursing program with active Florida RN license.
• Must maintain current CPR certification, valid driver's license, reliable transportation and current auto insurance.

Responsibilities

• Provide home-based care to patients in their homes, often after discharge from hospital, rehab or nursing facility.
• Coordinate with physicians and oversee LPNs, CNAs, and aides.
• Examine patients, perform assessments, document progress and report changes to physicians.
• Educate patients on plans of care, medications, therapy, diet and exercise.
• Coach family members on the plan of care, medication and home safety.
• Ensure goals are met and coordinate discharge from services.

In this role, you'll make a meaningful impact by:
