Job summary

Advantage Home Health in Carlisle, PA is seeking a compassionate Physical Therapist to join our PRN Home Health team. You will manage your own daylight schedule, provide one-on-one care in patients' homes, and help individuals regain mobility and independence in comfortable settings.

The role offers autonomy, per-visit compensation, and opportunities to pick up additional visits while contributing to meaningful patient outcomes and professional growth within a collaborative clinical team.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Pennsylvania.
• Current CPR certification (or ability to obtain upon hire).
• Strong communication, organizational, and documentation skills.

Responsibilities

• Provide skilled physical therapy services in accordance with physician orders and individualized plans of care. Evaluate patients and develop personalized treatment programs to improve function and quality of life. Assess mobility, strength, balance, and safety needs. Document patient evaluations, treatments, and progress in a timely and accurate manner. Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes. Educate patients and caregivers on exercises, safety techniques, and ongoing therapy goals. Supervise Physical Therapist Assistants as appropriate.
