Job summary

AccentCare seeks a Licensed Physical Therapist to provide evaluation and treatment for home health patients in the Clovis, NM area on a PRN basis. The role requires a CAPTE-approved PT degree, state licensure, CPR certification, and travel to multiple locations as needed.

The PT will perform evaluations, develop treatment plans, document progress, educate families, and coordinate with an interdisciplinary team while maintaining productivity and quality standards.

Qualifications

• Master's degree in physical therapy approved by CAPTE accredited physical therapist education program.
• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Currently licensed to practice as a physical therapist within the state of agency operation.
• Must possess and maintain valid CPR certification while employed in a clinical role.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
• Meets agency productivity standards and uses work hours efficiently.
