Job summary

Empath Health is seeking a compassionate PRN Occupational Therapist to join our Home Health team serving Venice and the South Sarasota area. You will conduct comprehensive assessments, develop care plans, and deliver skilled OT in patients’ homes with strong team collaboration and patient education.

Active Florida OT license and at least 1 year of OT experience required; home health experience preferred. Flexible PRN schedule with potential growth in patient caseload within a mission-driven

Qualifications

• Active Florida OT license.
• Minimum of 1 year experience as a licensed Occupational Therapist (OT).
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
