Job summary

Empath Health is seeking a compassionate Occupational Therapist to join our Home Health team in the Palm Harbor area on a PRN basis. The role involves delivering skilled OT services to patients at home across east Pasco County and surrounding areas with a flexible schedule of 5–10+ visits per week.

Qualifications

• Active Florida OT license.
• Minimum 1 year experience as OT.
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
