Empath Health, through Empath Home Health, is seeking a compassionate PRN Physical Therapist to serve patients in Port Charlotte, North Port, Venice and surrounding areas. You will perform assessments, design and revise plans of care, and deliver skilled PT in patients' homes.

This role offers flexible scheduling, weekend rotations, and strong interdisciplinary support with potential to become full-time as caseload grows.

Qualifications

• Active Florida PT license
• Minimum of 1 year experience as a licensed PT
• Home health care experience strongly preferred
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday rotations

Responsabilités

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled physical therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

Active Florida PT license
