Primecare Home Care is looking for a PRN LPN/RN Supervisor to carry out in-home supervisory visits for clients receiving home care services across multiple locations in Georgia. This role focuses on monitoring caregiver performance, assessing client conditions, and ensuring compliance with regulatory standards. Candidates must hold an active LPN or RN license in Georgia and possess strong communication and clinical judgment skills. Flexibility and reliable transportation are required for this independently performed role.
Qualifications
Home care or home health nursing experience preferred.
Supervisory visit experience preferred.
Familiarity with Medicaid and state home care regulations preferred.
Responsibilities
Conduct required supervisory visits in clients’ homes in accordance with state, Medicaid, and agency requirements.
Complete all supervisory visit documentation accurately and timely in the agency’s designated system.
Communicate visit findings, concerns, or recommendations to clinical leadership as required.
Skills
Strong written and verbal communication skills
Ability to provide professional feedback related to care delivery
Strong assessment and clinical judgment skills
Education
Active, unencumbered LPN or RN license in the State of Georgia
Current Basic Life Support (BLS) certification
Valid driver's license with reliable transportation
Current TB screening
