Job summary

Ascend Health is seeking a Registered Nurse - PRN for our Home Care team in Paramus, NJ. The RN will plan, organize and direct home care services and coordinate patient care in the home setting.

Education includes ADN with BSN preferred; must hold a State Certified RN license and have experience in Certified Home Care and OASIS. Candidate must be willing to travel and possess a valid driver’s license to serve patients in the community.

Qualifications

• Must have a State Certified RN license.
• Experience in Certified Home Care and OASIS.
• Willing to travel and have a valid driver’s license.
• Must be able to work PRN hours.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revises as patient status changes.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals and family involvement.
• Supervise LPN and HHA documentation of care and services.
• Provides health care instructions to the patient.
• Identifies discharge planning needs and implements prior to discharge.
• Reports incidents per agency policy.
• Communicates with the physician regarding patient needs and changes.
• Participates in case conferences and on-call duties.
