Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Fort Atkinson, WI. This PRN position supports the therapy team under licensed supervision to provide resident-centered OT services.

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings to residents and interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point of care documentation.
