Job summary

West Haven Healthcare is seeking a PRN Certified Occupational Therapy Assistant (COTA) to join our in-house rehab team. You will work closely with the interdisciplinary team to deliver care aligned with the plan of care and regulatory requirements.

The role involves patient education, home modification recommendations, and timely, accurate documentation, with collaboration across care providers to maintain high quality outcomes.

Qualifications

• Must have an OTA credential with a license in good standing.
• Associate's degree in Occupational Therapy Assistant completed.

Responsibilities

• Provide recommendations for home modifications and adaptive equipment with training as needed.
• Ensure all treatment delivered follows the established plan of care.
• Monitor patient response and adjust treatment with IDT collaboration.
• Provide clinical support and education to patients, families, and caregivers.
• Collaborate with the interdisciplinary team to ensure quality patient care.
• Complete documentation thoroughly and on time in compliance with company and state/federal regulations.
• Attend required meetings as designated by the Director of Rehab.
