Job summary

Wurtland Nursing & Rehab is seeking a Certified Occupational Therapist Assistant (COTA) to join our care team in Kentucky. The role supports delivering high-quality rehabilitative care under supervision of an Occupational Therapist, focusing on ADLs, mobility, and resident independence.

This PRN/as-needed position emphasizes collaboration with the interdisciplinary team, accurate documentation, HIPAA compliance, and compassionate geriatric care.

Qualifications

• Associate degree in Occupational Therapy Assistant from an accredited program.
• Current state licensure as a COTA in Kentucky.
• 1–2 years of geriatric care or SNF experience preferred.
• Familiarity with PDPM and SNF documentation is a plus.

Responsibilities

• Deliver OT interventions per supervising OT’s plan.
• Assist residents with exercises to improve ADLs and mobility.
• Monitor responses and report progress to supervising OT.
• Instruct residents and caregivers on adaptive equipment use.
• Maintain a safe, engaging therapy environment.
• Document sessions and progress per facility policies.
• Adhere to HIPAA, OSHA, and infection control standards.
