Job summary

Discovery at Home in Keller, TX is seeking a Certified Occupational Therapy Assistant to support evaluation and development of the rehabilitative plan under an Occupational Therapist, with home health experience preferred.

You will educate patients, document progress, and collaborate with other professionals to ensure comprehensive care. A COTA degree, state license, CPR, and valid driver's license are required for this full-time role.

Qualifications

Responsibilities

• Assist in evaluation and development of rehabilitative plan of care and re-evaluations.

Responsibilities

• Under the direction of the Occupational Therapist, assists in evaluation and development of the rehabilitative plan of care and in period re-evaluations as required.
• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
• Maintain accurate and timely documentation and progress notes.
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care
• Guide and instructs patients in prescribed therapeutic self-care and creative activities that are directed toward improving independence and functioning.
• Attend and participate in department staff meetings and case conference meetings.

Qualifications

• COTA degree from an accredited program
• A COTA license from applicable state
• One year of home health experience required
• Must have knowledge of Medicare and Medicaid guidelines.
• Current CPR certification
• Valid driver's license and reliable transportation
• Excellent verbal and written communication skills
• Basic computer skill proficiency. Ability to use Microsoft Windows.
