Job summary

The role emphasizes documentation and timely communication of patient status. Ideal candidates have a Home Care Aide Training certificate and CNA registration, plus a valid driver’s license.

Qualifications

• Certificate of completion of a state recognized Home Care Aide Training Program and supervised clinical practice.
• One year of experience working with adults in a healthcare setting.
• Current registration in the state Nurse Aide Registry as a Certified Nurse Aide.
• Valid driver’s license and automobile liability insurance.
• Home Health experience is desirable.

Responsibilities

• Provides competent nursing care to home health patients according to state and industry standards.
• Demonstrates knowledge of home care patient care skills.
• Helps maintain continuity of care by collaborating with the interdisciplinary team.
• Obtain vital signs of patients as assigned.
• Report patient progress and problems to supervisory personnel.
• Observe and report any changes in condition, including skin integrity.
• Provide comfort care related to turning and positioning.
• Maintain a safe, clean and comfortable environment for the patient.
• Assist with meal preparation and related tasks.
• Assist with patient transfer, ambulation and exercise.
• Provide personal hygiene to patients (bathing, shaving, oral hygiene, skin care, dressing, toileting, hair care).
• Maintain accurate and current records of visits.
• Communicate and document tasks in a timely and accurate manner.
