Job summary

Live Well Homecare Agency, LLC is seeking a Clinical Dietitian (PRN) to provide medical nutrition therapy as part of an interdisciplinary healthcare team. You will assess patients, develop diagnoses, and tailor care plans to support optimal outcomes.

Responsibilities include delivering nutrition education, monitoring interventions, mentoring interns, and contributing to quality improvement. RD/RDN certification and state licensure within six months are required.

Qualifications

• Bachelor’s degree in nutrition/dietetics from an accredited program.
• Registered Dietitian/Nutritionist (RD/RDN) and licensed within six months of hire.
• 1–2 years of clinical experience preferred, with proficiency in EMR and foodservice software.
• Excellent interpersonal, communication, and customer service skills.

Responsibilities

• Perform nutrition assessments, develop diagnoses, and create individualized care plans based on patient needs and best practices.
• Implement and evaluate nutrition interventions, including managing enteral nutrition support and transitional feeding plans.
• Provide nutrition education to patients, families, and healthcare team members on topics such as diets, supplements, and eating behaviors.
• Utilize the Nutrition Care Process, document patient encounters, and monitor outcomes for continuous improvement.
• Participate in research, quality improvement, and performance initiatives to enhance care quality.
• Mentor dietetic interns and maintain professional development through self-assessment and ongoing learning.
