Department: Care Management

Work Type: PRN

Shift: Shift 1 / 8:00 AM to 4:30 PM

Minimum to Midpoint Pay Rate: $40.86 per hour

Summary

Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs throughout the continuum in various settings through communication of available resources to promote quality, cost-effective outcomes. Provides discharge planning through assessment, referral, and interdisciplinary planning for high-risk populations, linking people to systems that provide resources, services, and opportunities to achieve and maintain maximum functioning.

Requirements

• Education: Associate's Degree in nursing with ongoing efforts to obtain a Bachelor's Degree.
• Experience: Minimum of 3 years clinical experience. Previous experience in discharge planning, utilization management, knowledge of community resources, assessment skills, and understanding of clinical and social factors affecting patients' functional status at discharge preferred.
• License: Currently licensed as an RN in the State of Florida.
• Certifications: Case Management certification (CCM, ACM, NACCM, or CMCN) required within 5 years of employment.
• Other: Strong communication and interpersonal skills, basic computer proficiency, analytical skills, good grammar and spelling, and the ability to effectively multi-task and prioritize responsibilities.
