Position Title

PRN Behavioral Health Case Manager - Emergency Dept Days/Nights, Bell Hospital

Position Summary / Career Interest:

We are currently recruiting for a PRN Behavioral Health Case Manager to join our Emergency Dept team! Both days and nights shifts are available.

The Behavioral Health Case Manager is responsible for providing safe and efficient care/services across a full range of patient ages and their families. The primary role involves collaborating, communicating, and facilitating service coordination during and after hospitalization, as established by the behavioral health care team and executed by the case manager.

Responsibilities and Essential Job Functions
  • Perform the professional, clinical, and technical competencies required for the assigned unit or department.
  • Accept responsibility and accountability for achieving optimal outcomes within their scope of practice, following policies, procedures, and standards. Address risk and safety issues associated with the position. Participate in on-call duties as required. Demonstrate accountability for actions and decisions.
  • Maintain knowledge and competence related to their position's expectations and scope of practice. Engage in departmental decision-making and keep current licensure.
  • Initiate and participate in family conferences to assess psychosocial and discharge planning needs.
  • Complete psychosocial assessments of patient/family situations, considering social, psychological, emotional, financial, and related factors to facilitate community reintegration.
  • Use social work assessments and team input to formulate realistic social work action plans during hospitalization and discharge planning.
  • Locate and connect patients with community resources to support their living situations.
  • Assist with financial concerns, including insurance issues, and apply for social security benefits and Medicaid/Medicare as appropriate.
  • Understand various State Waivers, their qualifications, and assist patients in determining eligibility and obtaining waiver services.
  • Connect patients with community mental health or other agencies for follow-up appointments, therapy, intensive case management, and other social services.
  • Coordinate community placements for post-hospital discharge, such as group homes or sober living arrangements, based on patient needs and safety.
  • Advocate for patients and caregivers to access necessary services, ensuring patient rights and a patient-centered environment.
  • Document all relevant information accurately in the patient’s medical record, including initial contact, assessments, daily contacts, family and discharge planning meetings, and all patient encounters within 24 hours.
  • Contribute to the hospital’s financial sustainability and work with the treatment team to ensure timely discharge using established procedures and resources.
  • Participate in case management activities, interdisciplinary team meetings, and provide coverage as needed.
  • Perform other duties as assigned by the Clinical Manager.
Required Education and Experience
  • Master’s Degree in Social Work
Required Licensure and Certification
  • Master’s level licensure with the Behavioral Sciences Regulatory Board, such as Licensed Professional Counselor (KS), Licensed Master Social Worker (KS), or Licensed Marriage and Family Therapist (KS).
  • State of Kansas Social Work license.
Knowledge Requirements
  • Proficiency in reading and documenting in electronic medical records is required.
