Job summary

Lafayette-Physical-Therapy-1 is a private outpatient clinic in Lafayette, CA, seeking a Physical Therapist to join a team that uses in-house diagnostics to identify root causes of conditions for the Lafayette area and eastern Bay.

Qualifications

• Valid California Physical Therapy license (or Pending) or PTLA status.

Responsibilities

• Evaluate and treat patients with a variety of diagnoses, establish treatment plans and goals.
• Coordinate with referral sources on patient progress.
• Discharge planning with physician, patient, family, and other facilities as needed.
• Document daily notes, patient evaluations, progress notes, re-evaluations and discharge summaries.
• Attend scheduled departmental meetings and training.
• Coordinate with other departments to ensure quality care.
