Job summary

Sacbar is seeking a skilled Physical Therapist to join our private outpatient orthopedic clinic in Lafayette, CA, serving the East Bay. We utilize in-house diagnostics and emphasize collaboration among clinicians to improve movement and function for patients.

Qualifications

• Valid California Physical Therapy License (or Pending) or PTLA status (Current or upcoming).
• New Graduates are welcome, with mentoring from senior therapists with diverse backgrounds.
• Experience in Geriatrics Rehab, Neurologic Rehab, and Orthopedic Rehab.
• Strong Manual Therapy Skills.
• APTA Membership and involvement encouraged, but not required.

Responsibilities

• Evaluate and treat patients with a variety of diagnoses, establish treatment plans and goals.
• Coordinate with referral sources on patient progress.
• Discharge planning with physician, patient, family, and other facilities as needed.
• Document daily notes, patient evaluations, progress notes, re-evaluations, and discharge summaries.
• Attend scheduled departmental meetings and training.
• Coordinate with other departments to facilitate quality care.
