Qualifications

• Credentialed in good standing with the Commission on Dietetics Registration
• Experience in one-on-one patient counseling
• Experience working in an outpatient setting and/or with wellness coaching

Responsibilities

• Provide comprehensive, evidence-based nutrition care to patients
• Conduct assessments and develop care plans using the Nutrition Care Process
• Monitor ongoing medical conditions and adjust care as needed
• Educate patients and families on nutrition and behavior change
• Collaborate with team to support program development
• Utilize EHRs and nutrition software in daily care
