Job summary

Interim HealthCare Inc. is seeking a Private Duty Nurse (RN/LPN) to provide in-home direct patient care and monitor progress according to the plan of care. You will document vitals, administer medications, and collaborate with care teams to achieve excellent patient outcomes.

This role supports flexible scheduling options (FT/PT/PRN) with competitive pay and benefits, and daily pay is available. A 1:1 patient care model is emphasized to ensure comfort and dignity for our clients.

Qualifications

• Graduate of an accredited school of nursing.
• Current unrestricted RN or LPN license in the state.
• CPR/AED/BLS/First Aid certification.
• Reliable transportation to care sites/work locations.
• Trach and/or ventilator experience preferred but not required.

Responsibilities

• Provide in-home direct patient care according to the prescribed plan of care.
• Assess and document patient conditions and report changes.
• Administer medication, insulin, and IV/fluids; document thoroughly.
• Inspect and care for wounds and assist with personal hygiene.
• Assess patients for injuries, infections, bedsores and deliver appropriate treatments.
• Collaborate with providers, aides, and health professionals to ensure patient outcomes.

As a Private Duty Nurse (RN/LPN), you will:

• Providein-homedirect patient care according to theprovider’sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery.
• Assessand document patient conditions and reportchangesas needed (vitals,blood/stool/urine, glucose,etc.).
• Administer medication, insulin, and IV/fluids;documenting thoroughly.
• Inspect and care for wounds, changing dressings, and assisting with personal hygiene.
• Assess patients for further injuries, infections, bedsores, etc., and deliver appropriate treatments.
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.

To qualify for a Private Duty Nurse (RN/LPN) with us, you will need:

• Education: Graduate of an accredited school of nursing.
• Licensure: Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position OR Licensed Practical Nurse (LPN).
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites and/or work locations.
• Practical trach and/or ventilator experience preferred but not required.
