Prairie River Home Care, Inc. in Minnesota is seeking a part-time Registered Nurse to deliver comprehensive in-home client care under the guidance of a Case Manager.

You will travel to client homes, assess needs, administer medications, educate families, and coordinate with the care team. Flexible evenings and weekend shifts are available, with travel across Mankato and Janesville areas; a valid MN RN license and reliable transportation required.

Formación

• Current MN RN license without restrictions.
• At least 2 years of healthcare experience.
• Acute hospital or home care experience preferred.
• Strong nursing judgment and teaching skills.
• Good interpersonal and communication skills.
• Ability to work independently with good organizational skills.
• Excellent written and oral communication skills.
• Access to a reliable vehicle or public transportation.
• Valid driver’s license and car insurance when using a personal vehicle.
• U.S. citizenship or valid Alien Work Permit.
• Background checks required.

Responsabilidades

• Plan, provide and evaluate nursing care per policies; consult with Case Manager on care plan changes.
• Observe, evaluate and document client care; maintain accurate records.
• Administer medications and treatments; provide education to client and family.
• Collaborate with families and other health professionals to ensure continuity of care.
• Assess symptoms and report changes to the Case Manager; adapt care as needed.
• Exercise professional judgment to modify nursing measures as needs change.
• Communicate effectively with the interdisciplinary team.
• Participate in scheduled client conferences.
• Maintain infection control practices and timely record keeping.
• Pursue ongoing professional growth and development.
