Qualifications

• RN degree as defined by state board.
• Current Texas State License as an RN (RN TX).
• Experience: minimum 6 months or 1 year as RN preferred.
• Strong nursing skills and professional demeanor; knowledge of medications.

Responsibilities

• Acts as liaison between patient/family and medical team.
• Performs patient evaluations by monitoring vital signs and medications.
• Develops day-to-day care plans for home care and hospital stay.
• Plans home-care health options for patients returning home.
• Obtains health history and documents current conditions.
• Adheres to safety and health code standards.
• Supervises licensed vocational nurses and PAS/PCS staff as needed.
• Lift up to 25 lbs.

Job Summary:

• Provides specialty care and services operating in a range of clinical settings.
• Assess, monitor and document patient’s needs and care.
• Collaborates with other health care providers to discuss patient care.

Essential Functions:

• Acts as liaison between patient and family and medical representatives.
• Acts as liaison between patient and family and medical representatives.
• Performs patient evaluations by monitoring vital signs, administering medication, placing IVs, etc.
• Develops day-to-day care plans for patients in hospital and once they are on home-care programs.
• Plans home-care health options for patients upon return home.
• Obtains health history information and documents current physical conditions.
• Practices most current safety and health code standards
• Supervises staffed licensed vocational nurses, PAS/PCS staff, as needed
• Able to lift 25 lbs
• Frequent standing, walking, sitting, stooping, kneeling or crouching
• Work independently and with team

Requirements

Job Qualifications:

Educations: Degree as a Registered Nurse

Licensure: Current Texas State License as an RN, current Texas Driver’s License.

Experience: Minimum 6 months’ experience or one year as a RN.

Transportation: Reliable transportation and valid and current auto liability insurance.
