Job summary

We Care Home Health is seeking a compassionate LPN or RN to provide private duty home care on a PRN basis, day or night shifts. You will care for a patient with complex needs, including trach, vent, and G-tube, in the patient’s home, ensuring safety, comfort, and stable medical condition.

Qualifications

• Current, unencumbered LPN or RN license.
• Experience with trach, vent, and G-tube care.
• Current BLS/CPR certification.
• Strong clinical and communication skills.
• Ability to work independently in a home care setting.
• Compassionate, patient-focused approach to nursing.
• Must work at least 1 shift/month to remain PRN on the case.

The Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide care for an extended care client on a PRN basis during day shift 0700-1900 or night shift 1900-0700. This is a private duty client with trach, vent, G-tube, and Hoyer transfer needs in addition to brief changes, bath cares, and med administration. You will be working in the patient's home, ensuring their safety, comfort, and medical stability.

What You'll Be Doing

• Delivering specialized care for a patient with trach, vent, and G-tube
• Monitoring the patient's respiratory and nutritional health during your shifts
• Administering medications and tube feedings
• Performing routine assessments, interventions, and adjustments as necessary
• Communicating with the care team and maintaining accurate patient records
• Developing strong, supportive relationships with the patient and their family
