Qualifications

• Current Washington State RN or LPN license in good standing.
• Active BLS certification.
• Strong assessment and clinical decision-making skills.
• Excellent communication and interpersonal abilities.
• Compassionate, patient-centered approach to care.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide one-on-one skilled nursing care in patients' homes and community settings.
• Perform regular nursing assessments and update care plans.
• Administer medications and treatments as ordered.
• Monitor patient status and report changes to the interdisciplinary team.
• Educate patients and families on treatments and disease management.
• Maintain accurate electronic medical records and ensure regulatory compliance.
