Qualifications

• Current Washington State RN or LPN license in good standing.
• BLS Certification is required or preferred depending on assignment.
• Strong assessment and clinical skills aligned with scope of practice.
• Excellent communication and interpersonal abilities.
• Compassionate, patient-centered approach to care.
• Valid driver's license and reliable transportation if assigned to multiple locations.

Responsibilities

• Provide one-on-one skilled nursing care in community and home settings.
• Perform ongoing nursing assessments appropriate to scope of practice.
• Implement individualized Plans of Care and document care activities.
• Administer medications and treatments as ordered; monitor patient status.
• Collaborate with physicians and interdisciplinary team to ensure quality outcomes.
• Educate patients and families about treatments, medications, and disease management.
• Maintain accurate EMR documentation and comply with regulations.
