Qualifications

• Active, unencumbered LPN/LVN license in the practicing state
• Current CPR certification
• TB skin test within the last 12 months
• Preferred: Six months of hands-on nursing experience
• Reliable transportation

Responsibilities

• Provide and document skilled nursing care according to care plans and physician orders
• Assess patient condition continuously and report changes
• Document all nursing activities timely and accurately
• Participate in updating and implementing care plans
• Ensure infection control measures are followed
• Educate patients and families on health management and prevention
• Maintain professional knowledge through ongoing education

Additional Requirements
