Guardian Angel Home Care is seeking a compassionate Licensed Vocational Nurse (LVN) to provide private duty nursing care for a patient in the San Jose, California area. The role offers a consistent Monday–Friday schedule from 11:00 AM to 5:00 PM, for 30 hours per week, in the patient’s home.

Qualifications

• Active California LVN license
• Graduate of an accredited vocational nursing program
• Current BLS/CPR certification
• Strong communication and patient care skills
• Ability to work independently in a private home setting
• Dependable and available for the assigned weekday schedule

Responsabilités

• Provide dedicated one-on-one skilled nursing care to a patient in their home.
• Monitor the patient's condition, vital signs, and overall health status.
• Administer medications and treatments as prescribed and within the LVN scope of practice.
• Assist with activities of daily living and patient care needs as appropriate.
• Observe and promptly report changes in the patient's condition to the supervising RN and physician.
• Maintain accurate and timely clinical documentation.
• Communicate professionally with the patient, family, and interdisciplinary care team.
• Follow the established plan of care and all applicable agency and state regulations.
