Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Provide 1:1 care for clients following the client's plan of care while maintaining their dignity and independence
• Administer medications, feedings, oxygen, ostomy care as needed, and assess results
• Accurately document nursing actions and communicate with family, case manager, and physicians; monitor progress and outcomes
• Coordinate or order supplies, medications, and feedings to ensure adequate stock
• Ensure all patient needs are met according to physician instructions

Qualifications

• LPN/LPT/LVN Degree from an accredited nursing program with current license in the state of practice
• Valid driver’s license, acceptable driving record, and proof of car insurance
• CPR certification
• 1+ years of nursing experience preferred
• Home health care experience preferred
