Responsibilities

Job Summary: The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality care within the geographic service area. She/he works under the supervision of the Registered Nurse or designated supervisor, complying with state laws, licensure requirements, and agency policies.

Specific Job Duties/Responsibilities:

• Provides high-quality clinical services within the scope of practice, following the plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record, utilizing the agency's electronic visit verification program.
• Participates in care coordination, reporting patient needs and changes, attending case conferences, and communicating effectively with patients, families, physicians, and team members.
• Maintains confidentiality of patient health information.

Education and Experience

License Requirements: Must have current LPN licensure in Tennessee, a current driver's license, vehicle insurance, access to a dependable vehicle or public transportation, and current CPR certification.

Experience Requirements: Previous home care experience preferred.
