Qualifications

• Graduated from an accredited school of professional nursing.
• Current BLS Certificate.

Responsibilities

• Demonstrate effective teamwork and collaboration with staff members.
• Monitor vital signs and progress; document observations.
• Perform nursing procedures per agency policies under RN supervision.
• Educate patients and families about plan of care and home care procedures.
• Assist with activities of daily living.
• Transport patients to appointments.
• Adhere to HIPAA and infection control policies.
• Document visits by end of each shift.

Education
