Job summary

BAYADA Home Health Care in Phoenix is seeking a compassionate Licensed Practical Nurse (LPN) to join our Skilled Nursing team in private duty roles for pediatric and geriatric clients, delivering one-on-one care with autonomy in the home.

Qualifications

• Graduation from an accredited nursing program as indicated by transcript or diploma.
• Active Arizona Licensed Practical Nurse (LPN) license in good standing.
• One year of experience as a Licensed Practical Nurse.
• Trach experience preferred, but not required.

Responsibilities

• Travel to a single client's home on a schedule you choose.
• Follow up with, execute, and properly document doctors' orders.
• Perform assessments and monitor clients' conditions.
• Document observations, interventions, and evaluations.
• Provide hands-on care with autonomy in the home.
